Hazel Hawkins to reopen San Juan clinic two days a week

To demonstrate our continued commitment to maintaining a healthy community in San Juan Bautista and to better serve their health care needs, the San Juan Bautista Community Health Center will re-open on Monday, April 27. 

The health center will be open two days a week, on Monday and Wednesday, from 8:00 am – 12:00 noon. Additional days will be added as patient volumes begin to return to normal.

Providers from the health care center are also doing Tele-Med calls with patients that need to be followed up on, but are not sick enough to come in, or need medication refills.

“Our goal is to keep our well patients safe at home, but still provide the care they need” said Linda Stroup, Director of HHMH Community Health Centers. “Please don’t hesitate to seek medical care.  We are still here for you.”

During the rest of the week, patients still have access to Tele-Med appointments and if patients need to be seen in person they will be referred to the Barragan Family Health Care and Diabetes Center at: 

930 Sunnyslope Rd, Suite A2, Hollister.

Realizing that transportation may be an issue for some patients, HHMH reached out to San Benito County Express which provided the following transportation information for San Juan Bautista residents.

Option # 1:

San Benito County Express service http://www.sanbenitocountyexpress.org/ The landing page shows what limited services they are running, which includes weekday Dial-a-Ride service from 6am to 6pm. To schedule a ride, passengers are asked to call a day in advance at (831) 636-4161.  At this time, fares are not being collected for the safety of their drivers and the clients.

Option # 2 more specifically for seniors and people with disabilities:

This specialized transportation service is operated by Jovenes de Antano http://jdasbcseniors.org/transportation The service they would be eligible for to attend medical appointments would be the weekday Medical Shopping Assistance Program, hours varying on the needs of the clients. They ask passengers to call at least 2 days in advance at (831) 637-9275 and are also not collecting fares.
